ERROR WAREHOUSE WAIVER AND RELEASE OF LIABILITY FORM

This Form is to be read in its entirety and signed prior to participation at ERROR WAREHOUSE ESCAPE ROOMS ACTIVITIES.

In regards to participating in the games and activities at ERROR WAREHOUSE ESCAPE ROOMS ACTIVITIES, I hereby waive, release and discharge from any all claims for damage for personal injury,or property damage which I may have hereafter accrued to me, as a result of my participation in the games and activates at ERROR WAREHOUSE HORROR ESCAPE ROOMS.(Room Massacre,Room Asylum,Room Djin).

This release is intended to discharge in advance ERROR WAREHOUSE HORROR ESCAPES ROOMS, its owners,managers,officers,employees,agents representatives and all associated parties from any and all liability arising out of or connected in any way with my participation in the games and activities at ERROR WAREHOUSE HORROR ESCAPE ROOMS.

I understand that in the activities offered at Error warehouse horror escape rooms may involve risks that might result emotional trauma,physical injury,death,permanent disability,maternal loss,bodily accidents from collisions,falls,contact with objects or walls and tumble ,medical conditions from exertion.

Knowing the risks,l here by assume those risks.l further agree to indemnify and hold Error warehouse horror escape rooms harmless from and loss,liability,injury,or expense which they incur as a result of any injury,property damage and risks I may sustain while am at Error warehouse facility and participate in any game and activity at the facility.

I fully understand that the nature of the activities is not suitable for pregnant women or people with medical conditions including but not limited to heart conditions, asthma, anxiety disorders, PTSD, lights or sound sensitivity and I shall Indemnify and hold the Releasees harmless for any injuries associated with participating at my own risk and responsibility.

I deliberately recognize and take on all inherent risks related to this event, including those arising from the Entities’ possible negligence. My engagement is wholly optional, and I choose to be a part of it while acknowledging these risks. If I deem any aspect of the experience unsafe or feel unfit to continue due to health concerns, I shall promptly withdraw.

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Releasees from any and all claims, demands, or causes of action which are in any way connected with my participation in this activity, or my use of their equipment or facilities, arising from negligence. This release does not apply to claims arising from intentional conduct. Should Releasees or anyone acting on their behalf be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

I affirm that I possess adequate insurance to address any harm or loss I might incur or cause during this event. If not, I commit to bear the expense of such damage or harm. I further confirm that no medical or physical situation could jeopardize my well-being during this event; otherwise, I willingly assume the associated risks.

I agree to observe all posted rules and warnings and to follow any oral instructions or directions given by the Releasees.

I am accountable for any damages to the Error ware house horror escape rooms premises resulting from my careless, reckless, or deliberate actions.

I accept that throughout my participation in the Error warehouse horror escape rooms activities , the escape room has the right to take images and video (including sound) footage of me, and that they may edit and use such photos, video, and/or sound materials for any marketing purposes and waiver the right to anti any royalties or other compensation arising from use of my image or video.

I fully acknowledge and I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

I agree that once I remit my entrance fee, under no circumstances will I be eligible for refunds.

The waiver will be interpreted widely in order to offer a release and waiver to the fullest extent permissible by law.

I CERTIFY THAT I HAVE READ AND FULLY UNDERSTAND THE CONTENTS OF THIS DOCUMENT. I AM AWARE THAT THIS IS A CONTRACT AND A RELEASE OF LIABILITY, AND I SIGN IT OF MY OWN FREE WILL. 

MY ONLINE SIGNATURE SHALL SUBSTITUTE FOR AND HAVE THE SAME LEGAL EFFECT AS IF I HAD SIGNED THIS WAIVER IN ORIGINAL INK WHEN REGISTERING ONLINE.
